What should I bring to my first appointment?
Your paperwork, insurance card,
co-pay and any x-rays, MRI's or CT scans.
How do I get my medical records?
Fill out a written request
that states we have permission to release your records. Fax the completed
form to 303-225-8129 or e-mail the completed form to
firstname.lastname@example.org. Once the
form is received, the records are processed and forwarded according to your
completed form. Please allow two weeks.
How do I get a refill on my medication?
Call your pharmacy and request a refill. Your pharmacy will call our refill
line. The refill line is for pharmacy's only. If it is a written
prescription, call 303-225-8120, select your doctor from
the menu and leave a message for his nurse/medical assistant.
What insurance plans do you accept?
We accept all insurance plans except Kaiser.

Do I need a referral?
Some insurance plans, mainly HMO's, still do require a referral from your
primary care physician. Most PPO plans do not require a referral. Please
check your insurance card or contact your insurance company if in doubt.

This amount will vary depending on your insurance plan and type of surgery
